Repeat violation under Official Notice EH17002134 issued 07 MAY 2026: a cockroach remediation plan was implemented over the summer in contract with a local Pest Control Operator, which is now in the ongoing monthly service phase. Convey this finding for targeted treatment by the contract PCO. (4 points deducted)
   • 4-602.114-602.11 Wash, rinse and sanitize contaminated surfaces of the food slicer as often as necessary to prevent the accumulation of soil, or at least every 4 hours during continuous use./ The food slicer was not cleaned after last use and was found stored with the blade, food holder, blade ring and blade guard heavily soiled with accumulations of dried, impacted food residues. Disassemble the slicer to its fullest extent, use cleaning implements and provide effective (and safe) manual cleaning to remove soils. Rinse; then rewash any surface with detectable residue after rinsing and before sanitizing. (5 points deducted)
