   • N.D.A.C § 33-39-01-03(6)Ice machines cleaned and sanitized — --Observation: Ice machines used on the premises must be cleaned and sanitized in accordance with the manufacturer's specifications at the frequency recommended by the manufacturer. Ice machine chutes had a mold-like substance inside. Clean all ice machine chutes.
   • N.D.A.C § 33-39-01-11(4)Operable exit lighting and signs — --Observation: Emergency exits must be indicated by operable exit lighting and signs. Emergency lighting was not operable at both stairway landings. Exit signs were not illuminated on the second floor north side and near Room 102. Maintenance was repairing the lights during the inspection.

Single-service items were stored directly on the floor. Store single-service items at least 6 inches off the floor.
   • 6-301.11Handwashing Cleanser - Availability — --Observation: Hand-cleansing soap shall be available at each hand sink. Hand soap was not available at the kitchen sink. Soap was provided during the inspection.
