   • K23RoachesNo rodents, insects, birds, or animals — A live nymph German cockroach was observed on a trap located near the back door of the janitorial sink which shares a wall with the kitchen. Couple of dead cockroaches were also observed on an old trap located below the two compartment sink. [CA] Conduct a pest control intervention immediately. A follow-up inspection shall be conducted on Tuesday, September 22,2026. If any live or dead cockroach is observed during the follow-up inspection, the school kitchen shall be closed.
   • K45Conducive conditionsFloor, walls, ceilings: built,maintained, clean — There is a gap between the screen door and the concrete floor in the back door. [CA] To prevent insect/rodent infestation, please install a weatherstripping on the screen door. The door leading to the janitorial room was left open. [CA] Keep the door closed to prevent vermin infestation into the kitchen.
   • K40Wiping cloths: properly used, stored — There was very low sanitizer concentration on one of the sanitizer buckets. [CA] Maintain the sanitizer concentration on the sanitizer buckets at 200 ppm quaternary ammonia. 100 ppm chlorine (bleach), 272 ppm -700 ppm lactic acid levels.
